Mechanic Q: pulsating when brakeing?

When I step on the brake it feels like I am steping on an oval or egg shape

works fine 98% of time then doesn't work or is about 20 mph behind

When driving 10 miles or so, and after heating up the transmission starts to shift erraticly when it shifts up or down when taking your foot off the gas pedal Its a deinate clunking sound
