Mechanic Q: speedometer?

Needle on instrument panel often won't register speed. Usually worse at speeds below 55 mph. I have replaced gear on transmission one year ago when speedometer failed completely. Some days it works ok & other days it works sometimes?

Makes noise especially when cold but not having any problems shifting.
