Mechanic Q: Van will rev high and move slowly, sometimes shaking violently, and can't go up small hills... ?

It began a few days ago with RPMs reving up to 4K, but the van moving very slow and taking a long time to build speed. It has progressed to the point where it comes to a stop on small hills, and begins shaking violently at slow speeds.
