Question: should the transmission be out of overdrive when not driving over 45 mph?

I've heard that it is better not to the transmission in overdrive when your driving less than 45 mph around town.

As long as you are not pulling a heavy load (or towing something), letting the transmission shift by itself is fine. Going up steep grades, it is better if the vehicle's engine is lugging to down shift out of overdrive, but in general it is fine to leave it alone. and you get better fuel economy.
