Question: Van will rev high and move slowly, sometimes shaking violently, and can't go up small hills...

It began a few days ago with RPMs reving up to 4K, but the van moving very slow and taking a long time to build speed. It has progressed to the point where it comes to a stop on small hills, and begins shaking violently at slow speeds.

You could need a new transmission. I would look at the fluid level to make sure it's full. If not add fluid and recheck. If full and burt looking you need a new transmission.
