Question: What is wrong with my van?

Hi, I have a 2005 ford freestar and it's having issues. After about 30-40 minutes of driving, if you're stopped and take off it has trouble accelerating. The rpm jumps to 3000-4000 rpms then it catches and starts picking up more speed. I just replaced the transmission thinking that was the problem but it's still doing it.

That does sound like it could be the transmission, or a part in the valve body. Who replaced it(a mechanic, a friend, you)? Was it a rebuild, new or used part? Are the shift points set properly? Have you had it scanned for codes?
